clinical case

This Patient attended wanting the spaces closed between her upper anterior teeth closed, as well as repairing the fractured incisal edges.

The following steps show the direct composite restoration using the Unica Proximal anterior matrix.

To maintain the ideal 80% height : width ratio of upper central incisors when closing spaces we need to lengthen (Magne 2003).

The cervical labio-palatal arch helps to secure the matrix between the papilla and the tooth sub-gingivally, in order to improve matrix stability after placement. A wedge can be used to secure the matrix but I find its not necessary when using these matrices.

The idea of using 2 back to back matrices when closing a space or even when placing 2 composite veneers, is simply to maintain the space for the adjacent tooth when restoring. It also acts like a wedge, which would normally compensate for the thickness of the matrix (30 microns). Therefore, the second matrix is acting as a wedge itself for separation.

conclusion

It is important to get the proximal area correct, not only for aesthetics, but for stability (prevent teeth drifting) and to protect the papilla from food trapping.
The use of the Unica allows the reproduction of the labial, palatal and proximal contours due to its pre-contoured shaped. Unlike posterior sectional matrices, they have longer cervical extensions for easy restoration of the cervical part of the proximal contour.

The Unica proximal cervical labio-palatal arch design also has the purpose of securing the matrix between the papilla and the tooth sub-gingivally. This makes the matrix very stable even without the use of a wedge.
