clinical case clinico
Class II (interproximal) decay involves the proximal surface of a posterior tooth and it is a common occurrence in many dental patients. One challenge for the clinician is to accurately recreate a physical contact to the adjacent tooth and, at the same time, to restore proper interproximal anatomic form. This case involves a teenager 16 years old with an interproximal lesion in his left lower molar (#36).
1: Pre-operative situation showing interproximal lesion on the lower molar #36.
2: A preoperative occlusal view after isolation.
3: The cavity after cleaning and disinfection.
4: Selecting a black matrix (Polydentia lumicontrast) it is possible to have better control of adaptation of the matrix on the margin thanks to its high contrast. To allow the matrix to embrace the tooth, we used myRing forte with specific extremities for young permanent molars, myTines small. If the embrace between the matrix and the ring is precise, we spend less time for corrections at the end of the restoration.
5: Selective etching of the enamel, 30 seconds. We proceed with bonding.
6: Construction of the mesial crest and light-curing.
7: Next step is to open the matrix and check if the margins we created are correct. Notice the ideal contouring of tooth perimeter.
8: We transform the class II restoration into a class and then proceed with finishing and polishing steps.
9: Occlusal view of the final direct restoration.
10: Contact point made with myRing Forte coupled with myTines small. In situations where the teeth have low crown heights, the use of a sectional matrix system with small ring extremities allows for better matrix adaptation leading to ideal contacts and contours.
conclusion
The possibility to customize our sectional matrix systems with the most suitable extremities gives us the advantage of recreating a good lasting contact point and ideal anatomy that allows easy care and cleaning by the patient.
